Local 500 ID Badge Renewal Procedure
Reminder:
Local
500 ID Badges are mandatory.
1)
For Renewing your Local 500 ID Badge

] request a copy of your
Hiring and Referral
Rules and Regulations.
Then
go to Forms &
Reports and download the
I-9 & W4.
Then continue with
the steps
below.
2)
Make sure all
I-9 & W4
forms are signed and completed
before you send them to prevent delays.
A)
Fill out and complete the I-9 & W4 forms.
Please make sure they are both signed.
B)
Please sign and return only page 14 of the
Hiring and Referral Rules and Regulations.
C)
You will need the following legible copies for your
I-9 Acceptable Documents as follows:
YOU MUST INCLUDE
(Note: All documents must be
unexpired)

U.S. Passport or
·
U.S. Passport Card
Or, both of the
following two documents below:

Drivers License and
·
Social Security Card
D)
Mail all completed forms with a self addressed stamped envelope.

to save yourself time, you may download all necessary forms above in
advance and bring the completed forms with you on your appointment
