Reminder:  Local 500 ID Badges are mandatory.
 

1)       For Renewing your Local 500 ID Badge  click [ HERE ] to download 
          your forms. Then continue with the next two steps below.

2)       Make sure all forms are signed and completed before you send them to
     prevent delays.

3)       Please mail all your completed forms and
   add a Self address stamped envelope to:
