Reminder: Local 500 ID Badges are mandatory.
You can renew your Local 500 ID Badge using one of the following methods:

Download, print and complete all forms and mail them along with a self addressed stamped envelope,
or
contact Local 500's office at (954) 202-2624 to schedule an appointment.

In order to save yourself some time, you may download the necessary forms in advance and bring the completed forms with you on your appointment date.
