2016 ID Badge Renewal Procedure
500 ID Badges are mandatory.
You can renew your Local 500 ID Badge using one of the following methods:

all forms and mail them along
with a self addressed stamped

bring the completed forms with you on your appointment date.
Payroll Service of Fla. is changing their payroll company and
EVERYONE MUST fill out a new hire package from Avitus Group
prior to working any Theatrical Payroll paid venues. The paperwork is available
via e-mail, our website under downloads or you can stop by the office to fill
out the forms.
